How to Become a Patient
hmo insurance plans
HMO insurance plans require a referral. Call your primary care clinic and request they fax a referral to us at 725-228-6427.
PPo insurance plans
A PPO insurance does not require a referral; however, we ask that you please fill out our new patient request form.
not sure or cash pay?
We take cash pay patients. If you are not sure the type of insurance you have, it’s best to contact your primary care for a referral just in case.
