New Patient Scheduling
Patients with PPO Insurance
If your insurance plan does not require a referral (typically a PPO plan):
- Complete our New Patient Request Form.
- Submit the completed form along with any relevant medical records through the submission button below.
Patients with HMO Insurance
If you have an HMO insurance plan:
Please have your Primary Care Provider (PCP) submit the following:
- Referral to Ologymed
- Relevant medical records
